Fever in infants 0 to 60 Days clinical pathways
Pathways pays for outreach and navigation, service coordination, case management, new housing support services, respite care, and the alignment of existing programs and data systems into a coordinated system of care. Enrollees will continue to receive on-going health care services paid for through Medi-Cal, while these other services are provided through Pathways. While the program can fund services to help individuals find and maintain housing, Medicaid prohibits paying directly for housing. Home health care is a wide range of health care services that can be provided in the comfort of your home for an illness or injury. Downtown Women’s Center was awarded $500,000 over three years to expand its model to support aging in place for older adult women residing in Permanent Supportive Housing . DWC’s work is influenced by the CAPABLE model, a program developed at the Johns Hopkins School of Nursing for low-income seniors to safely age in place. The grant will allow DWC to extend services to 245 additional units of PSH over three years and add RN and OT staff to serve aging women in these units.
Pathways to Health + Home
Our Hospital at Home programs are designed for hospitals that require urgent in-home medical assessments of patients who have had an exacerbation of an existing disease or injury. Our Skilled Nursing Facility at Home programs are designed to offer patients a choice of receiving their post-acute medical rehabilitation in the safety and comfort of their own home. The proof is in our outcomes, driven by our specialty programs and highly trained staff. Our innovative model is perfect for home health patients because it adds the unique feature of a physician organization.
A Shared Care Plan that identifies the enrollee’s unique needs and a treatment must be created for each enrollee within the first month of being in the program. The Shared Care Plan must be updated regularly by program staff, with each service documented every month. For enrollees receiving housing services, a housing specialist will assess the enrollee within 30 days and continue to meet weekly during the first six months of program enrollment.
Hospital at Home
Oftentimes these individuals do not seek care until it is an emergency. Pathways will also accept referrals from other homeless navigator programs serving the City based on eligibility criteria. Pathways uses evidence-based outreach and care coordination practices to ensure good outcomes for individuals experiencing, or at-risk of experiencing, homelessness. Individuals are referred by specific organizations, engaged in real-time by outreach workers, and enrolled in the program if eligible. Services are referred following an assessment that determines each individual’s unique needs. An inter-disciplinary team comprised of an outreach worker, care coordinator, housing specialist, and a clinician works closely with the enrollee, providing ongoing connection to stabilizing services and social supports.

Directions Health Services is one of the Canberra region’s most experienced community organisations delivering programs and services to people impacted by alcohol, drugs and other addictions. Pathway Home™ is a community-based care transition/management intervention offering intensive, mobile, time-limited services to individuals transitioning from an institutional setting back to the community. Although the location is changed, the vision and dream of William, Patricia and Judith will continue to unfold.
The basic foundation classes and principles will always be in place and the goal is to create that same sacred space where so many have found peace, healing and transformation. In home health, our goals are to help our patients improve their physical well-being, become as independent as they can be, and enjoy the best quality of life possible. Bereavement counseling and some other typically face-to-face services remain available via telephone and other media.

Pathways will serve a minimum of 3,250 individuals experiencing, or at-risk of experiencing, homelessness from November 2017 through December 2020. At full capacity, the program will have 1,000 individuals enrolled and receiving services on any given day.
In April, William, Patricia and Judith first opened Pathways to Health in Nesconset, NY and the vision became a reality. In that first year they jointly created over 12 empowering classes and certification courses. Spirit Journey became the foundation class and AN-RA energy based healing was introduced. Pathways Of Love & Light ( a healing multi-faith ministry est. 1993 by William Bezmen ) also grew and they began an ordination tract with their first ministers, Patricia Stringham, Ed Stringham and Judith Grant.
The introduction and forward tells the story of what brought them together in 1995. Pathways was guided to a new location in Mt.Sinai, NY and converted a home on 3 acres as the healing center. More teachers were certified and ministers were ordained during that year.
Maintains a positive attitude and willingness to share expertise to new clinical provider or office staff. Notifies Home Health Aide Department daily of intended visit route and visit times. Communicates promptly and appropriately with patients and families regarding scheduled visits. Assists in training individuals or families in the care of terminally ill patients. Emergency and urgent care locations
The center is staffed by a multidisciplinary team of New York State licensed and certified health care and education professionals, with referral services to a cadre of other health care providers, educators, and consultants. A successful transition from inpatient care often demands navigating a complex and fragmented health system. Pathway Home™’s goal is to ensure uninterrupted and coordinated access to behavioral and physical health services while addressing the social determinants of health. CBC acts as a single point of referral to multidisciplinary teams at ten care management agencies in CBC’s broader IPA network. These teams maintain small caseloads and offer flexible interventions where frequency, duration and intensity is tailored to match the individual’s community needs and have the capacity to respond rapidly to crisis.
Pathways provides a wide range of health care services that can be provided in the comfort and safety of your home. Certified home health care services are ordered by a physician, require skilled care, and is typically paid for by Medicare or other health insurances plans.
